Can Stress Cause Shortness of Breath and Chest Pain?

Stress and anxiety can cause shortness of breath and chest pain, but similar symptoms may also point to heart or lung issues. This blog explains red flags, when to seek care, and how ARC Network helps provide answers through testing.

If you’ve ever felt shortness of breath, chest tightness, a racing heart, or sudden fatigue and wondered, “Is this anxiety… or is something wrong with my heart or lungs?” know that this is a common question. Those symptoms are some of the most common reasons people seek urgent care, visit the ER, or book follow-ups with their primary care provider.

Stress and anxiety can absolutely cause physical symptoms, including chest discomfort and breathlessness. The important part to note is that sometimes similar symptoms can point to an underlying heart, lung, or sleep issue, and getting clarity is worth it.

We’re breaking down how stress affects breathing patterns, how hyperventilation can mimic lung disease, when shortness of breath is NOT “just anxiety,” and the red flags that require medical assessment.

First: if you’re worried, trust that instinct

Before we talk about stress and anxiety, it’s worth stating clearly:

If you have new, severe, or persistent chest pain or shortness of breath, especially with sweating, nausea, light-headedness, or pain spreading to the arm/jaw/back — call 9‑1‑1 right away.

In Alberta, you can also call Health Link at 811 for non-emergency advice and next steps when you’re unsure.

Can stress cause shortness of breath and chest pain?

Yes. Stress and anxiety can trigger a “fight-or-flight” response that affects your breathing and cardiovascular system, sometimes quickly.

When stress spikes, people commonly experience:

  • Shortness of breath (dyspnea)
  • Chest tightness or chest pain
  • Racing heart / pounding heartbeat
  • Dizziness or light-headedness
  • Tingling in hands/around the mouth
  • Fatigue (often made worse by poor sleep)

These symptoms can feel intense, and because they overlap with heart and lung symptoms, it makes sense that people worry.

How stress affects breathing patterns and why it feels scary

Under stress, many people shift into faster, shallower breathing, often from the chest instead of the diaphragm.

Here’s the tricky part: when you breathe faster than your body needs, you can end up feeling like you can’t get a satisfying breath, even though you’re breathing a lot.

This can create a loop:

  1. Stress or worry starts
  2. Breathing speeds up
  3. Physical symptoms appear (tight chest, dizziness, “air hunger”)
  4. Those symptoms feel alarming, which increases stress
  5. Breathing speeds up more

If you’ve experienced this, it isn’t “all in your head.” It’s a real body response.

How hyperventilation mimics lung disease

When people hear hyperventilation, they often picture dramatic panting, but it can be subtle.

What hyperventilation is

Hyperventilation is rapid and/or deep breathing where you exhale more than you inhale, and it can happen due to stress or anxiety (though physical conditions can also contribute).

Why it causes chest tightness, dizziness, tingling, and breathlessness

Over-breathing can lower carbon dioxide in the blood, which can trigger symptoms such as light-headedness, chest tightness, fatigue, tingling, and feeling out of breath.

This is one reason anxiety episodes can feel like a lung or heart emergency, and the symptoms are real and uncomfortable.

If you suspect stress breathing in the moment

Some commonly recommended approaches include slowing the breath and breathing more gently (for example, pursed-lip breathing).

Important: if this is new, severe, or you’re not sure what’s happening, get medical guidance first.

When shortness of breath is NOT “just anxiety”

Stress can absolutely trigger symptoms, but it can also exist alongside conditions that need treatment.

Signs it may be more than stress (lungs)

Shortness of breath that is persistent, recurring, or associated with symptoms like chronic cough, wheeze, frequent sputum, or getting winded during everyday chores can point toward respiratory conditions where lung testing is appropriate.

ARC Network provides Pulmonary Function Testing (PFT), a group of tests that measure how well your lungs move air and exchange gases.

Signs it may be more than stress (heart)

Chest discomfort and breathlessness that is new, worsening, persistent, or triggered by exertion should be medically assessed, even if stress is present.

ARC Network offers cardiology consultation and diagnostic services in Alberta.

Diagnostics include tests such as ECG, echocardiogram, exercise stress testing, Holter monitoring, and BP monitoring.

Signs it may be more than stress (sleep)

If fatigue is part of the picture, especially ongoing tiredness that doesn’t improve with sleep, it may be worth asking whether sleep is restorative. ARC Network offers sleep testing and sleep consultations, including full sleep studies and a portable sleep monitor.

If obstructive sleep apnea is diagnosed, ARC Network provides treatment support, including CPAP therapy education and equipment guidance.

Red flags: when to seek urgent or emergency medical care

Chest pain and shortness of breath should always be taken seriously.

Call 9‑1‑1 immediately if you have signs of a heart attack

Heart and Stroke Foundation guidance includes calling 9‑1‑1 for symptoms such as:

  • Chest discomfort (pressure, squeezing, fullness, burning, heaviness)
  • Shortness of breath
  • Sweating, nausea, light-headedness
  • Upper-body discomfort (neck/jaw/shoulder/arms/back)

Seek medical assessment soon (even if symptoms come and go) if:

  • Symptoms are new, worsening, or happening at rest
  • You’ve been told you have risk factors and symptoms are changing
  • You keep thinking, “Something feels off,” even when stress is clearly present

If you’re unsure and it’s not an emergency, Alberta’s Health Link 811 is a helpful first step.

Anxiety vs. heart/lung issue: A Quick Pattern Guide (Not a Diagnosis)

Only medical assessment and testing can confirm what’s going on, but patterns can help you decide how urgently to seek care.

Patterns that can occur with anxiety/panic

  • Symptoms that come on during acute stress, with rapid breathing and sensations like tingling or dizziness
  • A sense of “air hunger” that may be linked to over-breathing

Patterns that should raise concern for heart/lung causes

  • Chest discomfort with sweating, nausea, light-headedness, shortness of breath, or pain spreading to the arm/jaw/back
  • Persistent or exertional symptoms, or symptoms that are new/different for you

If you’re not sure, it’s safer to get assessed than to assume it’s “just anxiety.”

How ARC Network Helps You Get Clarity (Without the Runaround)

When you’re stuck in the question — “Is this stress or something wrong with my heart or lungs?” — objective testing can be incredibly reassuring.

ARC Network offers several pathways to answers across Alberta, including respiratory, cardiac, sleep, and oxygen services.

1. Pulmonary Function Testing (PFT) for Shortness of Breath

PFTs measure how well your lungs move air and can help assess causes of breathlessness. PFTs are used to diagnose lung disease (such as asthma/COPD), assess shortness of breath, and evaluate treatment progress.

Common Questions About Stress, Breathing and Chest Pain

Can stress cause shortness of breath even when you’re resting?

Yes. Stress and anxiety can change breathing patterns and trigger rapid or deep breathing (hyperventilation), which can create a real sensation of breathlessness.

Can anxiety cause chest pain that feels like a heart attack?

Anxiety and panic can cause chest tightness and pain, and symptoms can overlap with cardiac issues, which is why medical assessment is important when symptoms are new or concerning.

What does hyperventilation feel like?

Hyperventilation can cause symptoms such as chest pain/tightness, dizziness, tingling, and feeling out of breath.

When should I call 9‑1‑1 for chest pain and shortness of breath?

Call 9‑1‑1 if you have chest discomfort with warning signs such as sweating, nausea, light-headedness, shortness of breath, or pain spreading to your arm/jaw/back.

What tests check lung function for shortness of breath?

Pulmonary Function Tests (PFTs) measure how well the lungs take in and release air and move gases, and are commonly used to help assess causes of breathlessness.

What cardiology tests might be ordered for chest pain or palpitations?

ARC provides cardiology diagnostic services such as ECG, echocardiogram, exercise stress testing, Holter monitoring, and BP monitoring.

Could sleep apnea cause fatigue and related symptoms?

ARC provides sleep diagnostics (full sleep studies or portable monitoring) and treatment pathways when obstructive sleep apnea is diagnosed, which is commonly associated with fatigue.

You don’t have to sit with the uncertainty

Stress can absolutely cause shortness of breath and chest tightness and knowing that can be reassuring. But if symptoms are new, recurring, or worrying, you deserve clarity and peace of mind — and the right testing can help you get there.
